By signing this form, the applicant (you) hereby waives all claims to a Homeowner Exemption for the tax year indicated, for the property described by the Property Index Number indicated. This form may be submitted entirely online. After you submit this form it will be received by Cook County Assessor's Office staff for review.

IMPORTANT: in 2020, exemption applications and tax bills are processed for Tax Year 2019. It is important to supply the correct Tax Year.
